High blood pressure, gout and restless legs

SB was 43 when he presented with a history of hypertension. His doctor indicated that if his blood pressure was not controlled then he would need to have it managed pharmaceutically. He was keen to avoid medications, as he was already taking drugs for gout, as well as benzodiazepines and painkillers for a back injury. He was concerned the he was overweight and that his liver enzymes were elevated. He was having trouble sleeping at night due to Restless Legs Syndrome (RLS) and was experiencing joint pains associated with gout. Despite going on long fast walks during the week and following an intermittent fasting diet but he was unable to lose weight. First Consultation Protocol

In this case, hypertension and weight gain, the chief presenting complaints, don’t require direct intervention. Rather, an investigation of root causes led to the identification of oxidative stress and poor detoxification as the likely causes of subtle nutrient deficiencies that contribute to high blood pressure and weight gain.

Gout and RLS are associated with high oxidative stress and chronic inflammation, which can lead to increased weight and hypertension. Gout develops when high concentrations of uric acid deposit as crystals in the joints. Under normal physiological conditions, the liver produces uric acid which acts as an antioxidant and provides over 50% of the antioxidant capacity of blood plasma. However when antioxidant capacity is low, uric acid rises, representing an attempted protective response in the body. At high concentrations uric acid can function as a pro-oxidant, correlating and predicting the development of obesity, hypertension and cardiovascular disease.

RLS is associated with the oxidation of dopamine in the brain which leads to impaired neurotransmission for generating movement. RLS increases the risk for cardiovascular disease and hypertension.

Antioxidant depletion may be caused by a number of factors including: a high alcohol intake, heavy metals such mercury in fillings, toxic elements in vaccinations, infections, pharmaceutical drugs, a low intake of vitamins, minerals and phytonutrients, and stress.

The protocol focused on increasing the antioxidant status of the body, however for antioxidant supplementation to be effective, an optimum level of digestion and assimilation is required. Given suspected hypochlorhydria, bile and intestinal permeability issues, these were dealt with first, in Phase I of the protocol. Increasing antioxidants will increase detoxification and production of oxidants in the liver, hence elimination and excretion pathways must also be addressed. In particular, gastrointestinal permeability can lead to an increased level of toxins entering enterohepatic recirculation, causing more stress to the liver. Additionally, dietary changes are recommended to support the protocol.

The protocol aimed to:

  • optimise the gastrointestinal tract (GIT) function for digestion, assimilation and excretion;
  • optimise liver function for biotransformation and elimination;

SB made excellent progress in 2 weeks. He adhered closely to his new nutritional plan, in which he was well supported by his wife who took all the recommendations on board when planning meals.

SB’s weight reduced by 2kg and his blood pressure lowered from 163/113 to 133/93. He was not having any sugar cravings or feeling hungry. His RLS and gout symptoms decreased significantly so he slept much better. His shoulder and back pain reduced from 8/10 to 5/10 and so he reduced his intake of painkillers and benzodiazepines. Four weeks after starting the protocol SB lost 4kg in weight and his blood pressure further reduced to 120/87. His blood work showed normal uric acid production, normal liver enzymes, normal cholesterol and ferritin levels. SB’s doctor was happy for him to stop all his medications and reported that his stroke risk had reduced from 19 to 9.6.

SB reported that his energy levels were high, and despite being very busy at work and home, he was coping very well. He was having several days in a row without any pain at all, and when there was shoulder or back pain, it was very much reduced. RLS and gout symptoms had completed resolved.

Antibiotics, fungal infections, asthma and digestive issues

Sue was a 47 year old journalist living in Scotland and working from home. She reported fungal infections for the last 14 years after the birth of her first child, prior to which she reported the use of antibiotics for childhood ear infections. First Consultation Protocol

Plan rationale: Sue was aware that a high carbohydrate diet can promote fungal growth, and she thought that by switching from obviously high sugar foods such as biscuits and cakes to healthier foods such as fruits and oat bars, she would reduce her sugar intake. In fact these ‘healthy’ foods still contain high amounts of sugar if eaten in high quantities. A high sugar diet can destabilise regulation of blood sugar, and this can lead to energy highs and lows in the daytime, particularly in the afternoon.

We agreed on a diet that focused on increasing proteins, beneficial fats and vegetables, reducing carbohydrates. Sue was encouraged to eat 3 meals a day and avoid snacking.

Sue was using Salbutamol for asthma, and this may have depleted calcium, magnesium, phosphate and potassium. Sue was advised on a number of supportive supplements.

As the infections had been going on for a number of years, SR agreed to perform a comprehensive stool analysis which can identify multiple microbial infections and digestive and immune functionality in the GIT.

SR responded well to the protocol. Her energy levels improved and she felt more able to work in the afternoons. Her fungal infections, digestive symptoms and diarrhoea showed some improvement, and she lost some weight.

The results of SR’s stool test had come back from the lab and were as follows:

  • Bacteriology culture indicated dysbiosis – no growth of Enterococcus, low level of E. coli and high levels of Streptococcus. Streptococcus overgrowth in the gut can be problematic as it acts as a reservoir for reinfection leading to sinusitis, and produces D-lactic acid which can lead to fatigue. Excess lactic acid can also disrupt the normal pH of the gastrointestinal tract, making it overly acidic, as seen by the pH result. This may damage the mucosal barrier of the intestine, leading to increased gut permeability and the leaking of bacterial by-products and partially digested food proteins into the blood which may cause allergies.
  • Yeast microscopy identified the presence of small amounts of yeast. Streptococcus can potentiate Candida, amplifying the severity of infection.
  • Fat stain was positive indicating fat maldigestion, as reported by SR. This may may due to low bile production and/or deconjugation and reduction of bile acids by Streptococcus bacteria. Deconjugated bile acids are injurious to the gut epithelium and may cause diarrhea.
  • Lactoferrin and calprotectin were within range, indicating a lack of appropriate immune response, as both have anti-pathogenic activity. Immune markers were high as would be expected indicating inflammation, however Candida has the ability to inactivate immune system antibodies and so lowers the body’s immune response.

Analysis of the test results indicated that the presence of Streptococcus was likely to be enhancing the effects of Candida, hence both pathogens needed to be addressed at the same time. Eradication of pathogens may cause a die off reaction and unpleasant side effects if the the gut is permeable, hence a protective amino acid was recommended to support the gut lining. SR’s immune response was low hence supplementation with colostrum was advised. Colostrum is rich in immunoglobulins which provide passive immunity against pathogens. Sue presented with a long standing fungal infection which began following the birth of her first child. During pregnancy the mother’s immune system switches to Th2 arm of the immune system and should switch back to Th1 post birth. If however she has remained Th2 dominant then there is likely to be a pro-Candida bias, and further, Candida enhances Th2. Antibiotic use can cause an imbalance in the bacterial flora of the digestive system which can further enhance Th2 bias. Antibiotic use may have lead to strep overgrowth and the subsequent diarhoea. Often the detrimental effects of Candida are an allergic reaction to the yeast as well as from a reaction to its toxins. The Th1 cellular response is crucial to controlling Candida and supplementing with colostrum can be beneficial to shift the cellular response.

The comprehensive stool test was very useful in this case, showing that focusing solely on Candida eradication would not have had the desired impact, as the presence of Streptococcus was playing a part in the entrenchment of the infection. Dealing with both pathogens lead to a higher likelihood of eradicating the fungal infection.

Acid production by the stomach and bile synthesis in the liver play a large role in eradicating pathogens in the gastrointestinal tact. Optimising liver and digestive function not only protects the body from infections, but also allows more efficient extraction and assimilation of nutrients, which are required for all of the body’s cellular functions.

Candida infection is common, particularly in women, where it can flourish in the presence of high oestrogen. This case demonstrates that in some instances there may be additional pathogenic infections as well as imbalances in digestive, immune and liver function, hence focusing solely on Candida may not the whole answer. I recommend comprehensive stool testing for all digestive complaints, as the results can provide so much information for a targeted protocol to finally overcome long-term symptoms.

Stomach ulcers

Diana was 36 years old when she presented with stomach pain, indigestion and bloating after meals. We agreed on a health optimisation plan that included a natural anti-bacterial agent, as well as targeted supplements for her sluggish digestion, vitamin and mineral depletion, high stress levels and lack of energy:

The first priority was eradicating the parasite, and mastic gum has shown to minimise Helicobacter pylori infections. The infection may have lead to malabsorption of nutrients from the gut, hence a transdermal multivitamin and multi mineral spray was employed to bypass impaired digestion and assimilation in the digestive system. High stress levels were modulated with Rhodiola rosea, a botanical adaptogen which has shown to modulate cortisol levels. My Review

Conventional medicine treats Helicobacter infections with multiple rounds of antibiotics, starting with a course of triple antibiotics. This results in eradicating the infection but leaves the patient vulnerable to subsequent infections and additional courses of antibiotics, leading to collapse of the gut microbiome and the possibility of severe digestive symptoms. Here the infection was eradicated with a gentle anti-bacterial and stress modulation prevented reoccurence.

Psoriasis

Dave was 32 when he came to see me about the psoriasis that he had been suffering with for almost 10 years:

Root Cause Analysis

Psoriasis is a chronic auto-immune skin condition that causes red, flaky, crusty patches of skin covered with silvery scales. Here the immune system triggers T cells to drive inflammation which results in hyper formation of skin cells. Additionally Dave began experiencing arthritic symptoms 7 years after the onset of psoriasis. Psoriatic arthritis can be extremely disabling and Dave was concerned as worked as farmer and gardener.

Every autoimmune condition requires 3 factors:

  • a genetic predisposition
  • intestinal permeability (otherwise known as a leaky gut)
  • one or more triggers

The environmental triggers and mediators of psoriasis include:

  • physical and/or psychological trauma
  • infections

““I’d been living with persistent psoriasis for a decade or so. I’d become complacent, but when I started developing arthritis (which I assumed to be psoriatic), I decided to do something about it, and a friend recommended Sandra.

My Review

It was immensely gratifying to be able to help Dave find his way to resolving his psoriasis and arthritis. These chronic conditions involve multiple biological systems and are impacted by environmental factors such as stress, nutrition and infections. At 59, Linda arrived with severe brain fog, low energy and a clear goal: to feel strong enough to train for a trek. Rather than chasing each symptom separately, we mapped how stress, blood sugar and her intracrine hormone system were interacting – and worked the pattern, not the parts.

Lifelong depression and severe premenstrual anxiety

OT was 33 years when she got in touch with me seeking help for her long-standing daily anxiety and depression, which became much worse before her period. She felt low 80% of the time and even worse in the week before her period, where she would have an emotional meltdown. She had tried all kinds of approaches to manage her low moods including counselling, anti-depressants and hormonal birth control, none of which were successful.

“I have suffered with anxiety and low grade depression for as long as I can remember although I only realised that there was a name for how I felt when I was in my late teens. Whilst at college, I hit my lowest point and had some counselling but I think i only had a few sessions. The counselling did not work for me as it felt very “standardised” rather than looking to help me work through my own personal issues. I struggled with my mental health all through university but managed to distract myself with friends and work.

Recently I made positive changes to my diet, but I still suffer with anxiety and depression. I started taking sertraline as I again hit rock bottom. I stopped taking sertraline around 8-12 months ago as I was having bad side effects including severe night sweats. My anxiety and depression is also heightened a week leading up to my period and I am now looking for a more natural way to manage my symptoms as I hate how I feel most of the time. This state of being can be so frustrating at times as I know that I am very fortunate in my life but my mood is always low.

I believe my menstrual cycle has an effect as I have a few days around the time I am ovulating where I feel “normal” or better than I usually do.”

Root Cause Analysis

OT’s mood was low in general, which indicates a deficiency of serotonin and dopamine, and this was confirmed by a neurotransmitter questionnaire.

Depressive, anxious syndromes can be caused by nutritional deficiencies from poor diet, including

  • 5HTP derived from animal protein which creates serotonin
  • tyrosine derived from animal protein which creates dopamine.
  • magnesium
  • calcium

Additionally, it was very clear that her low mood was cyclical, improving at ovulation when oestrogen is higher, and crashing premenstrually. It was also clear that her oestrogen was too low in general and progesterone was too high premenstrually, as evidenced by

  • light periods
  • short periods – 2-3 days
  • breast tenderness
  • vaginal dryness

Factors contributing to low oestrogen in OT included

  • inadequate fat intake: steroid hormones such as oestrogen (and vitamin D) are made from cholesterol which is derived from dietary fat, which she could not digest and absorb
  • inadequate B vitamins which prevents the synthesis of oestrogen from testosterone

Follow-up Consultation 1 Results (+4 weeks)

  • 4 weeks later OT, for the first time, did not have the usual pre-menstrual emotional meltdown of breaking down and crying, and was feeling emotionally well during the month.
  • She did have 2 short depressive episodes, but said that at the same time her workload was excessively stressful.
  • Her anxiety had improved slightly, and she was still finding it difficult to fall asleep.

  • 4 weeks later and all of OT’s symptoms had resolved.
  • Again she had no breakdown before her period and her day-to-day emotional life was absolutely fine.
  • She had a couple of anxious moments related to work stress, but that was all.

My Review

Despite the fact that I see significant improvements in client’s health every day in my work, it still sometimes takes my breath away when I see the power of functional medicine in action.

I always expect to see results within 4 weeks, and OT’s progress was no different. She was committed to the process right from the start and did an incredible job of following her plan.

As she said to me, and I agreed – “I wish I could have done this years ago, and I dread to think what my life would have been like if I hadn’t done this work with you”. Acne and severe PMS

BA was 33 years old and working as a solicitor when she came to see me saying:

“Since the age of 13 I have been suffering from what I think is cystic acne. At that age, once every 6 months or so, I would get one massive spot on my face. Health History

  • Cystic acne started a 13
  • She was prescribed Lymacyline (a tetracyline antibiotic), Clamidamice (an antibiotic which is given to treat serious bacterial infections), Roaccutane (a vitamin A derivative with multiple side effects), and Yazmin (a contraceptive pill). None of these were effective and her acne was getting progressively worse.
  • 20: began a stressful career and started feeling anxiety, low patience, irritability, feeling angry, emotional and tearful. She had bouts of severe mood swings and uncontrollable anger.

Root Cause Analysis

Acne

  • Acne is mediated by high testosterone levels, stimulated by insulin resistance/spikes mediated by high sugar/starch intake, and dairy via Insulin Growth factor.

PMS

  • BA’s oestrogen was too high as evidenced by heavy bleeding and ovulation pain.
  • Excess oestrogen symptoms additionally include premenstrual anxiety, acne and mood swings.
  • Vitamin D is vital modulator of immune and hormonal function, and black women are particularly susceptible to deficiency. Low levels of vitamin D increase susceptibility to PMS.

Premenstrual Dysphoric Disorder reversed in 8 weeks

Tanya was 43 when she came to see me about what she described as severe premenstrual syndrome (PMS):

  • Prior to having children her PMS only occurred 1 day before her period, where she was ‘scatty and snippy’. Since having the twins 3 years ago it had got much worse and over that last 2 years it had become severe.

Root Cause Analysis

Tanya described her symptoms as severe PMS, but I suspected that she was suffering from premenstrual dysphoric disorder (PMDD). Both PMS and PMDD are associated with disturbing, predictable, recurring, cyclic symptoms that start towards the end of the cycle and stop shortly after the onset of menstruation. Common symptoms include headaches, breast tenderness, bloating, irritability, depression, angry outbursts, and anxiety. The difference between PMDD and PMS is the severity and predominance of mental/emotional symptoms in PMDD and the marked impairment of personal/social relationships, including increased conflicts. PMDD is at the most severe and disabling end of the spectrum of premenstrual disorders.

Tanya experienced at least 5 of the diagnostic criteria required for PMDD including 3 of the depressive, anxiety, cognitive, and physical symptoms which seriously interfered with her social activities and personal relationships.

The exact cause of PMDD is unknown, but it appears to be related to an abnormal response of neurotransmitters including serotonin and GABA to normal ovarian function. In PMDD there is no demonstrable hormonal imbalance, rather PMDD is the result of the way that oestrogen and progesterone a!ect serotonin and GABA metabolism.

PMDD usually starts within 1-2 weeks before the onset of menstruation, when progesterone is climbing and oestrogen is decreasing. Progesterone is converted to alloprogesterone which normally interacts with GABA receptors in the brain to produce feelings of calmness and relaxation. In PMDD however, it is thought that the GABA receptor is malformed which means that the alloprogesterone does not lead to calmness, and instead results in high anxiety, anger and other mental/emotional symptoms. This is probably why Tanya had such an extreme reaction to the synthetic progesterone she received during IVF treatment – the very high level of progesterone induced extreme PMS symptoms. The malformation of the GABA receptor is associated with various nutritional deficiencies, including vitamin K, magnesium and vitamin B6.

At the same time, the decreasing oestrogen level in the luteal phase is associated with a decrease in the level of serotonin. In PMDD this can be hightend, leading to a serotonin imbalance which can cause depression, anxiety, sugar cravings, insomnia, headaches, obsessions and compulsions.

Serotonin is not found in food, but it is synthesised from l-tryptophan, which is found in protein as an amino acid. Synthesis of serotonin requires vitamin B6 and vitamin D. Tanya had vitamin D deficiency as evidenced by her blood test, which likely contributed to her ‘winter blues’ and may have impacted her serotonin level and led to insomnia. Melatonin, the sleep hormone, is synthesised from serotonin, and hence sleep is worsened when vitamin D is low.

Tanya’s diet, which was high in sugars and starch, and low in vegetables, fibre, omega 3 fats and water was also a likely contributor to her symptoms. A high carbohydrate intake depletes the body of essential vitamins and minerals, while a lack of omega 3 fats and vegetables can lead to deficiencies in essential anti-oxidants and micronutrients.

Since having had my children, my hormones were all over the place and my mood swings were out of control. I did not recognise myself or my actions throughout certain times of the month and after a while I finally plugged up the courage to go and see my GP to talk about the symptoms I was experiencing.

The help which I was offered was one of three options:

  • To take the mini pill
  • Go onto anti-depressants

    My Review

    I was so pleased for Tanya and the quick turnaround she made in her health that benefited not just her, but her family as well.

    PMDD is severely emotionally debilitating condition. My approach is based on research that shows there isn’t an issue with hormone levels but in the the problematic way that progesterone acts at the brain level. My resolution of metabolic and neurotransmitter pathways at the brain level was key here.

    In 8 weeks Tanyas emotions had completely calmed down and she was able to take on all of life’s ups and downs without dreading her period or feeling desperate. She is now able to maintain emotional stability throughout her cycle..

    We agreed that Tanya will have brief check-in in 3 months time to monitor her health and determine her dosage of vitamin D for the winter months.
